Use several forms on one Shopify template
Add Form Builder Pro app sections to a product template and choose which product pages show each form.
Yes — you can place several Form Builder Pro forms on the same Shopify template and choose which product pages show each one. Each form is added as an app section in the theme editor, then restricted to specific URLs.
Add a form section to a template
- Create the form in Form Builder Pro.
- In Shopify admin, go to Online Store → Themes and click Customize on your current theme.
- In the theme editor, open the page selector at the top, choose Products and pick the product template you want.
- In the left panel, click Add section, open the Apps tab and choose CB Form Builder Pro.
- In Form Builder Pro, open the form's Installation page and copy the Form ID and CB public key.
- Paste both values into the app section's settings in the theme editor.
Show the form on specific products only
In the same app section, use Show on page URLs only to limit where the form appears. Enter the path of each product page, one per line. For example, to show the form only on https://your-domain.com/products/product-a, enter:
/products/product-a
Repeat these steps to add more forms to the same template, each restricted to its own products or pages. Click Save in the theme editor when you're done.
